Ibu dapat melihat contoh suratnya pada Lampiran VIB Per-159 Tahun 2006. Untuk lebih jeasnya Ibu bisa baca di Pasal 9 ayat 4 Peraturan DJP yang sama. Kalau ada tambahan atau ralat dari yang lain. Terima kasih
